The athletic trainers at DeMatha are available for the recognition, care, prevention, and rehabilitation of sports-related injuries, basic first-aid procedures and injury evaluation concepts, and procedures in the use of modalities and taping techniques.
Trainers introduce students to the components of physical conditioning as a means to prevent athletic injuries and to promote health and performance. They are available during the school day and at most events. In addition, Ms. Norris teaches a course in Sports Medicine.

Athletic Trainer System (ATS)
We have an exciting new program through the Sports Medicine Department! The ATS online portal will provide an easier method of submitting medical documents, communicating with the athletic training staff, and keeping track of your student-athletes daily progress through injuries.
Each student will be required to have an account through the website below. The account will be interactive with the athletic training staff, parents, coaches and if necessary, teachers. Please follow the directions listed to the right of this information under “ATS” to setup an account for each of your student-athletes.
Four (4) forms need to be printed, completed, and uploaded to the the ATS site PRIOR to the son's participation in tryouts, practices or games:
Health Examination Form
This form needs to be filled out and signed by the son's physician. It must be completed, signed, and dated by the student's doctor after June 1, 2016.
Medical History Form
This form needs to be filled out and signed by the student's parent or guardian.
Student Emergency Form and Safe Sports School Form
These forms need to be filled out and signed by the son's parent or guardian.

Once you have reviewed the above information, please sign the form included in the health forms packet and upload to ATS.
The Athletic Handbook needs to be reviewed by the athlete and their parent or guardian. Upon the completion of reviewing the handbook, page 23 along with Appendix A, Permission Form and Release (listed below under forms), needs to be printed, signed, and turned in along with a $30 check (weight room fee) to your Coach before the below dates:Fall Sports: Aug. 28 (Varsity Football: Aug. 21)
Winter Sports: Nov. 20
Spring Sports: Mar. 1
Permission Form and Release (Appendix A in Athletic Handbook)
Every athlete MUST have this document signed with all the required signatures for each sport they play. For example, if the athlete plays soccer and baseball, they will need TWO separate forms completed and signed. These forms are given to the Head Coach of the participating team.
DeMatha Transportation Verification (CARPOOL)
(Appendix B in Athletic Handbook)
This form MUST be completed if an athlete is carpooling with another athlete for EVERY time they are carpooling. The form must be given to the Head Coach prior to the date the athletes are carpooling.
DeMatha Transportation Verification (RETURN TRIP WITH PARENTS) (Appendix C in Athletic Handbook)
This form MUST be completed if the athlete is not returning on the school provided transportation after a game (if being provided). This form must be complete EVERY instance an athlete is not returning on school provided transportation after a game. The form must be given to the Head Coach prior to the athlete leaving the game site.
Athletic Injury Referral
If a student-athlete sees a physician for an injury or illness, a note must be provided to the athletic training staff. Please have the form “ATHLETIC INJURY REFERRAL” completed. This form is located to the right of this information.
